Description of Services:
Community health care agency * Personal Support Manager available to complete free in-home assessment to establish care plan that meets patients needs to remain independent in own home
  • bathing/assistance with personal care
  • dressing assistance
  • mobility assistance
  • meal preparation assistance

Nursing services provided in designated clinic locations.  Please call our office for further information.  Nursing care provided in clinics:

  • wound care
  • IV therapy
  • catheter
  • drain/staple/suture care
  • health teaching regarding diagnosis/disease self-management/medication management

Nursing in-home visits for patients who meet the clinic exclusion criteria

TB testing available * call 1-800-443-4615 ext 3246 for Grey Bruce/Huron Perth clinic locations, or call 1-800-443-4615 ext 3200 for Oxford/Elgin locations, and for fee information and appointment booking details.
