Description of Services:

Community health care agency * Personal Support Manager available to complete free in-home assessment to establish care plan that meets patients needs to remain independent in own home

bathing/assistance with personal care
dressing assistance
mobility assistance
meal preparation assistance

Nursing services are available at various clinic locations

Please call our office for clinic information. Clinic nursing services provided include:

wound care
IV therapy
catheter care
drain/suture/staple care
health teaching regarding diagnosis/disease self-management/medication management

Nursing care in-home where patients meet the exclusion criteria to attend clinic
