Services provided in client's home or designated clinic locations

Description of Services:

Community health care agency * Personal Support Managers available to complete free in-home assessment to establish care plan that meets patients needs to remain independent in own home.

In-Home Personal Support Workers provide:

Bathing/assistance with personal care
Dressing assistance
Mobility assistance
Meal preparation assistance

Nursing services are provided in designated clinic locations. Please call our office for further information. Clinic nursing services include:

Wound care
IV Therapy
catheter
Drain/staple/suture care
Health teaching regarding diagnosis/disease self-management/medication management

Nursing visits in-home are provided for patients who meet the clinic exclusion criteria.
