Description of Services:
Programs and services for people with mild cognitive impairment, Alzheimer's disease and other dementias and their care partners

  • Education Programs and Services Support programs and services for people living with Alzheimer's disease and other dementias, Mild Cognitive Impairment (MCI), and their care partners.
  •  
  • Finding Your Way ID Kit Finding Your Way Ontario provides public education and an ID kit to assist in a search for missing persons. With consent, ID kits are completed by registered clients and faxed to local police for entry into their database, so all relevant information about the person living with dementia is already stored and ready to be used in the event of an emergency. Contact your Social Worker or contact@alzswp for more information.

  • In-home Recreation Program Regular visits to people with Alzheimer’s disease and other dementias, for respite and companionship by trained volunteers

  • Support Services One-on-one support and group supports for people with mild cognitive impairment, Alzheimer's disease and other dementias and care partners

About Us

The Alzheimer Society Southwest Partners offers a wide range of programs and services designed for individuals with dementia and/or memory concerns, their family, care partners, and health professionals, as well as the general public. At AlzSWP, both the individual and the family are provided with support upon diagnosis and throughout the continuum of the disease.
