Description of Services:
To provide supportive housing for individuals who are homeless or at risk of becoming homeless and who have a mental illness requiring supports to build independent living skills necessary to maintain housing. The program works with private market landlords in the community acting in a liaison capacity to assist with providing access to modest safe, affordable and stable housing for consumers who require community support services. This program provides:
Supportive housing provider with rent subsidy
Community based private market housing
Search for private market safe, affordable and stable housing
Landlord/tenant resolution and tenancy support
Supports to build independent living skills and maintain housing in the community
