Description of Services:
Satellite office providing assessment and treatment for people with substance use, problem gambling/behavioural, and/or mental health concerns. 

Services offered include, but are not limited to:
  • Goal setting and support with treatment planning
  • Advocacy
  • Education and resources about living with mental illness
  • Referral and connection to various community services
  • Support engaging in activities of daily living while managing mental wellness
  • Connection to in house addiction services
  •  
  • Court Diversion Program and Court Support Services Partners with the criminal justice system and the mental health system to provide direct service to people who have a serious mental illness, developmental challenges, or brain injury who have been charged with a minor offence * program promotes treatment and rehabilitation to prevent recidivism
  •  
  • Mental Health Case Management Works with individuals experiencing symptoms of mental illness who require support to live and work in the community * case managers provide on-going support as needed in order to stabilize, achieve goals, and improve quality of life * case management services are tailored to the individual, based on their identified goals for support * participation in the Case Management Program is a prerequisite to obtain CMHA Huron Perth supportive housing

  • Peer Outreach Services Peer Outreach Workers use their own lived experience with mental health and addiction to walk alongside people in their journey of recovery and wellness * supportive listening and assist with navigating the mental health, addiction, and health care systems
