Self-management education and exercise rehabilitation program for individuals living with pulmonary disease. Comprehensive assessment by a Registered Nurse(RN)/Respiratory Therapist (RT) and Kinesiologist; home safety assessments as needed; individualized education and therapeutic exercise plan; counselling and support services
