Application
Referral Process: Program eligibility is assessed and determined by Rehabilitation Charge Nurse based on the ESC Home and Support Services Rehabilitative Care Committee’s Standardized Access and Discharge Criteria. Referrals can be initiated within the Meditech order entry system for internal Bluewater Health referrals. For external referrals call Patient Flow at 519-464-4400 ext. 5355 between the hours of 8:00AM to 4:00PM. A phyician's referral is required.

Description of Services:

Program Description:
Patients on this unit have defined rehabilitation goals and are cognitively and physically able to participate in a time-limited, coordinated inter-professional rehabilitation plan of care ranging from low to high intensity through a combined and coordinated use of medical, nursing and allied health skills.  Patients may be medically complex with long-term illnesses/disabilities.  On admission, patients may have limited physical and/or cognitive capacity due to medical complexity but are believed to have restorative potential.
 
Objectives of the Program:
- Rehabilitation is focused on enabling, individuals with impairments and disabilities to reach and maintain their optimal physical, sensory, intellectual, psychological and social functional levels

-Individual rehabilitation goals are achieved

-Return home or to an alternate environment
 
Who provides service:
An Interprofessional team including medicine, nursing, occupational therapy, physiotherapy, speech language pathology, social work, pharmacy, clinical nutrition, stroke clinical nurse specialist.
