Description of Services:
Program designed for persons at high risk for Hepatitis C infection due to drug use and those who are Hepatitis C positive who have complex addiction or social issues
Outreach workers, peer support workers, nurses and social workers support all aspects of clients life - physical, emotional, psychological * referrals to other community agencies * screening, prevention, care and treatment of Hepatitis C
