Provides assessments, counselling, discharge planning services, information and education

Patients can counsel with social workers about:

adjustment to illness and treatment
relationship and family issues
stress and conflict
grief and loss
end of life care issues
parenting issues
discharge planning, making connections to hospital and community-based resources, and home supports
communication needs and issues with the health care team
issues related to physical, emotional and sexual abuse, and neglect
