Description of Services:
Case Management workers offer short-term, goal-oriented services to improve mental health, quality of life and opportunity to work towards recovery * includes life skills training, community support and advocacy for people who have a serious mental illness
Includes the following:
coordination of care and services
group programs for skills development and social and recreational activities
health education about mental disorders and use of medications
relapse prevention planning and strategies
referral and advocacy services to other community agencies or partners
