Description of Services:
Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse, dietitian and a nurse practitioner on staff
Outreach clinics at Bruce Power, Chippewas of Nawash Unceded First Nation, Saugeen First Nation and the Southwest Ontario Aboriginal Health Access Centre
Diabetic Foot Ulcer Clinic
Assessment and treatment of diabetic foot ulcers * assessments by a Nurse Practitioner with access to a multi-disciplinary wound care team * specialized devices to take pressure away from the foot ulcer offered at no charge
Insulin Pump Program
Team of registered nurses and registered dietitians provide education and support for adults and children with type 1 diabetes who are using or considering using insulin pumps and continuous glucose monitors
Pediatric Program
Team of registered nurse, registered dietitian and social worker provides education and coaching in self management to children with diabetes and their families
