Description of Services:
Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
  •  
  • Dining for Seniors Group meals, free blood pressure clinics and social activities offered in various locations * call for locations
  •  
  • Exercise and Wellness Focuses on seniors maintaining and/or improving their overall health
  •  
  • Foot Care Basic and advanced foot care provided by a nurses certified in advanced foot care management * foot care includes care of corns and calluses, nail cutting, ingrown nails, thickened nails, advice on proper foot wear needs, and foot care education
  •  
  • Friendly Visiting and Telephone Reassurance Friendly Visiting - Regular volunteer visits to older adults and people with disabilities who live alone or are identified as someone who would benefit from this program * volunteers are matched one to one with older people
  •  
  • Home At Last Program Assists older adults with transitioning from hospital to home
  •  
  • Home Help Assistance with daily chores, such as laundry, light housekeeping, heavy cleaning, yard work, errands, shopping, and light meal preparation * Home Help staff visits are arranged to work with the client's schedule and preferences, can be daily, weekly, bi-weekly, or monthly

  • Meals on Wheels Hot meals - Delivered Mon-Fri between 11:30am-1pm * special diet types available

  • Personal Support and Homemaking Services Assistance older people and people with disabilities who are not eligible for Home and Community Care Support Services South West Region services and need help with daily chores, such as laundry, light housekeeping, and home maintenance activities * home help visits weekly, bi-weekly, monthly, or seasonally

  • Transportation - EasyRide Non-emergency transportation to local medical appointments and regular activities such as shopping, errands, and social activities * volunteer drivers * wheelchair accessible vans, cars, and taxis
