Provides a range of programs for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
Adult Day Services
Adult day program * social, recreational, and therapeutic activities for older adults, adults with special needs, and adults with Alzheimer's disease and other related dementia in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
Assisted Living
Provides on-site personal support services to eligible individuals
Exercise and Wellness
Programs include blood pressure checks (CHAP - Cardiovascular Health Assessment Program), health education, group exercise classes, in-home one-on-one exercise programs * held in various locations * view agency website for locations and times
Foot Care
Basic and advanced foot care provided by nurses with a certification in advanced foot care at community clinics
