17-bed acute care hospital with emergency department

Medical, surgery, therapy and ambulatory care services * care coordinators from Ontario Health atHome South West area available on site

  • Diabetes Grey Bruce Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring:
    • blood sugar levels
    • medications
    • healthy eating
    • exercise
    • other lifestyle choices 
    Staff include:
    • registered nurse
    • registered dietitian
    • nurse practitioner (all certified diabetes educators)
  •  
  • Outpatient Clinics Outpatient specialist clinics providing diagnoses and treatments for various medical conditions
  •  
  • Outpatient Physiotherapy Community physiotherapy clinic * health professionals assess and treat conditions that affect the muscles, nerves, and bones that may be caused by injuries, chronic conditions, or to recover after an orthopaedic surgery
  •  
  • Palliative Care Provides supportive end-of-life care
