Description of Services:
Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services

Provides comprehensive community health, support and in-home services by nurses, personal support workers and volunteers

  • Assisted Living Care for older adults with chronic illness or cognitive impairment who require assistance with independent living
  •  
  • Caregiver Support Circle Peer support groups, one-on-one support, education, and discussion for caregivers * referrals to community services
  •  
  • Cool-Aid Program Clients receive a magnetized pouch to keep on the refrigerator that contains places to record relevant information so it is accessible for emergency personnel in the event of an emergency
  •  
  • Falls Prevention and Group Exercise Classes for Seniors Group exercise and falls prevention classes for older adults and people with mobility issues and chronic conditions * aims to improve strength and balance to prevent injury and falls
  •  
  • Home At Last Program Assists older adults returning home after an overnight hospital stay.
  •  
  • Supportive Care Program - Hospice Visiting and Bereavement Support Services Hospice volunteers provide companionship, respite care, and emotional support to people and their family/caregivers dealing with a life-limiting or life-threatening illness * bereavement support for anyone who has experienced the death of someone close * services can be provided at home, or in hospitals and long-term care homes * palliative care education

  • Telephone Reassurance Regular phone calls between 8:30am-1pm weekly to check on the health and safety of older adults and adults with disabilities who live alone * screened volunteers listen, talk, and help isolated people stay in contact with the outside world

  • Visiting Nursing Visiting nursing care which may include assessment, treatment and education of wound and ostomy care, education for people with diabetes, pain and symptom management, palliative care, and TB skin testing (only in Owen Sound)
