Stroke Care and Outcomes in CCC and LTC Report 2018
Of the approximately 13,000 Ontarians who survive an acute care hospitalization for stroke or transient ischemic attack each year, about 1,100 are admitted to complex continuing care (CCC) and 1,300 to long-term care (LTC) within 180 days of discharge from acute care. To date, the care provided to stroke survivors in these settings has been largely unexamined.1-4 This report aims to address this knowledge gap by describing the sociodemographic characteristics and burden of care of stroke survivors admitted to CCC and LTC in Ontario between 2010 and 2015, the nature and extent of the rehabilitation therapy available to them, selected stroke best practices and outcomes, and their journeys through the health care system
