CorHealth Ontario is a provincial agency formed to provide strategic leadership to improve cardiac, stroke and vascular care. CorHealth Ontario provides a strategic understanding of the care needs of both patients and providers as the basis for clinical quality improvement, provincial planning, resource allocation and performance measurement. They advocate for services that are safe, effective, patient-centered, timely, efficient and equitable across the province.

CorHealth Ontario is now part of Ontario Health, an agency created by the Government of Ontario with a mandate to connect and coordinate our province’s health care system to help ensure that Ontarians receive the best possible care.

Although SWOSN helps lead regional and local quality improvement, each stroke district in Southwestern Ontario has a District Stroke Manager who champions best practices and quality improvement along the entire continuum of stroke care.
