Description of Services:
MASTER YOUR HEALTH:
6-week education series of workshops for individuals living with chronic conditions, developed by Stanford University and delivered worldwide. Clients who have completed the program experience: significant improvements in exercise, cognitive symptom management, improved self reported general health, health distress, fatigue, disability, and activities of daily living in addition to fewer days in hospital, outpatient visits and hospitalizations.

POWERFUL TOOLS FOR CAREGIVERS:
A 6-week series of workshops offering
caregivers the ability to develop a wealth of self-care tools. Caregivers learn how to
reduce stress, take care of themselves, and increase their ability to make tough decisions.
DIABETES, HEALTHY FEET & YOU:
A 2.5 hour, interactive workshop providing support and education on foot care.
Participants learn how to assess the impact of diabetes on the health of their feet, identify factors that can lead to foot ulcers and learn how to care for their feet daily.
GETTING THE MOST FROM YOUR HEALTHCARE APPOINTMENT:
A 1.5 hour, interactive workshop focusing on what to do before, during and after an appointment and uses action planning to optimize people’s health. This workshop prepares and empowers individuals to communicate more effectively with their healthcare team.
