Short term program, temporary bundle of transitional support services intended to assist older adults returning home from a hospitalization including an Emergency room trip.

This free bundle of services includes:

Clients receive up to 4 days of frozen meals; a ride home from hospital and up to 2 rides to follow up medical appointments; a weekly visits for 4 to 6 weeks for light housekeeping, laundry, meal prep or grocery shopping assistance, and; connections and referrals to community supports for on-going support.
