Vision, Mission, and Values

 

The Southwestern Ontario Stroke Network (SWOSN) is 1 of 11 stroke networks in Ontario providing leadership and planning to implement best practice stroke care across the continuum of care.

As part of the Ontario Stroke System, our vision is FEWER STROKES.  BETTER OUTCOMES.

Our mission is to continuously improve stroke prevention, care, recovery and re-integration.
This is achieved by collaborating with partners to re-organize stroke care delivery to ensure that all Ontarians have access to appropriate, quality stroke care in a timely manner.

In 1997, the Heart and Stroke Foundation of Ontario convened a group of stroke experts from across the province to develop a framework for stroke care in Ontario, launching the Coordinated Stroke Strategy.

In 2000, the Ministry of Health and Long-Term Care supported the development of the Ontario Stroke System. The goal of the stroke system is to decrease the incidence of stroke and improve care and outcomes by reorganizing stroke care to ensure that all Ontarians have access to appropriate, quality stroke care in a timely manner.

The Ontario Stroke Network (OSN) was created in 2008 to provide provincial leadership and planning for the continuum of stroke care in Ontario. Its key function is to enable the advancement of the Ontario Stroke System through leading provincial initiatives, acting as a credible advisor, evaluating and driving system performance and coordinating research and knowledge management programs.
