The Huron Perth Healthcare Alliance is made up of 4 hospital sites: St. Marys Memorial Hospital, Clinton Public Hospital, Seaforth Community Hospital and Stratford General Hospital. The District Stroke Centre is located at Stratford General Hospital in the city of Stratford and provides access to stroke care for both Huron and Perth Counties.
Stratford General Hospital is the largest hospital for Huron and Perth counties offering specialized services for stroke survivors. The hospital was designated a District Stroke Centre in 2002. Protocols for the administration of tPA, a clot busting drug for stroke as well as an EMS Destination protocol are in place for patients that experience an ischemic stroke and qualify for this therapy within a 3 hour window. A stroke prevention clinic provides services to patients that experience a Transit Ischemic Attack that provides access to physicians with stroke expertise and early diagnostics. The 15 bed Rehabilitation Unit is a full service unit that provides Occupational Therapists, Physiotherapists and Speech Language Pathologists, and nurses that are experts in the care and recovery of stroke survivors.

- Huron County has a population of 59,700 and Perth County a population of 73,765 for a total population of 133,375.
- Huron and Perth County have a higher percentage of seniors over age 65 compared to the rest of the province. (Stats Canada 2001).
- Cardiovascular disease in males in Perth County is statistically significantly more than the rest of Ontario while Huron County rates are at the Ontario average (Non-communicable Disease Surveillance Info base, Public Health Agency of Canada 2001). A higher percentage of males in Perth die from cardiovascular disease than the Ontario average while a higher percentage of females die of strokes in Huron County.
- Of the clients seen in the stroke prevention clinic 72% had a sedentary lifestyle; 69% were hypertensive; 66% had a BMI > 25; 45% were diabetic and 34% were smokers.
- The estimated number of clients with diabetes is 9336. A total of 110 patient were discharged from Huron Perth Hospitals with a diagnosis of Diabetes in 2004-2005. As of January 2005, 2015 clients with diabetes were registered in the Diabetic Education Program in Perth and 2247 clients in Huron County.
- There were 170 patients admitted to Huron-Perth hospitals with a stroke diagnosis in 2004 2005 fiscal year. There were 1721 patients admitted to hospital with cardiovascular diagnosis in Huron and Perth Counties in the same fiscal year.
