Programs and Services:
The Grey Bruce District Stroke Centre partners with a variety of health and community agencies to ensure best practice stroke care across the continuum. Ambulance by-pass, acute care, inpatient rehabilitation, and community care are available. Health promotion and prevention activities are achieved in partnership with the Grey Bruce Health Unit and through participation in a district alliance – the Grey Bruce Healthy Living Partnership. Community agencies and Long Term Care Facilities work closely with the District Stroke Centre to ensure best practice care and community re-engagement. Together with its partners, the District Stroke Centre is developing a coordinated approach to chronic disease management.
The Secondary Prevention Clinic is located at Grey Bruce Health Services – Owen Sound site. The clinic serves individuals who have had a transient ischemic attack or are at high risk of stroke, through assessment, diagnostics and treatment including risk modification.
