Nestled in the heart of Southwestern Ontario between Lake Erie, Lake St.Clair and Lake Huron, Chatham-Kent has a population of 107,000 within 23 communities of various size and character – agricultural, rural, small town and small city – which make up the municipality. The Chatham-Kent Health Alliance is a proud partnership of 3 hospital corporations located on 2 campuses – Chatham and Wallaceburg. Our model of care is patient and family centered. We provide primary, secondary and specialized services serving the medical needs of people in the Municipality of Chatham-Kent, South Lambton and Walpole Island. The municipality of Chatham-Kent provides the expertise of several community health agencies in the delivery of quality care such as with the eight Long-Term Care facilities, sixteen Residential Accommodations, Erie St.Clair CCAC, Public Health Unit/Heart Health Coalition and the local Heart and Stroke Foundation.

Programs and Services:
In April 2003 Chatham-Kent was designated as one of the five District Stroke Centers within the Southwest region of the province. Coordinated efforts provide Best Practice stroke care strategies and ensure that stroke survivors and their families experience continuity of care as they move through the stroke system. Chatham’s participation in the telestroke project BRAINSAVE, which builds capacity for the assessment and treatment of acute stroke using videoconferencing and tele-radiology, the implementation of the Integrated Stroke Unit within Rehabilitation/CCC program, and the continued dedication to the provision of an integrated, interdisciplinary approach to primary and secondary stroke prevention have all been embraced by our community. The Stroke Prevention Clinic continues to work collaboratively with other vascular health models to provide prevention leadership within our community, district and region.

  • For years, Chatham-Kent has been acknowledged as a hot spot for circulatory problems.
  • Statistics Canada figures show that Chatham-Kent has Ontario s highest per-capita death rate from heart attacks, and Canada s second highest per capita death rate from the same condition.
